Here are some thoughts/ideas (in no particular order): 1) Open up the cylinder and check if the ejector rod has come loose. It’s reverse-threaded, so turning it clockwise will loosen it. I got 3 FTE's in the first 200 rounds and chalked it up to break-in. (Although I have only had an issue with break-in one other time and it was not Ruger).
