I think there was a problem early in the season with it and they fixed it later on, but you need to dismiss all the operators you have and start from scratch for the exfil streak to start counting correctly. What I do to fix it is retract one of your operators, rehire an operator on that slot, do 1 exfil streak, then go back to the operator with a broken exfil streak, and it will be working again next time you exfil. 2.
